These terms govern your use of our website and telehealth services. By booking an appointment, you agree to the terms outlined below.
1. Acceptance of Terms
By accessing esaletterforhousing.com or booking a telehealth visit, you agree to be bound by these Terms & Conditions. If you do not agree, please do not use our services.
2. Description of Service
We connect patients with licensed physicians for telehealth evaluations related to Emotional Support Animal (ESA) and Psychiatric Service Dog (PSD) documentation. Whether a letter is issued is determined solely by the evaluating physician based on clinical judgment.
3. Eligibility
You must be at least 18 years old and a resident of, or physically located in, a state where our physicians are licensed to provide telehealth services at the time of your visit.
4. Medical Disclaimer
Our services do not replace ongoing care from a primary care physician or mental health provider. See our full Medical Disclaimer for details.
5. User Responsibilities
You agree to provide accurate, truthful information during your evaluation. Providing false information to obtain a letter may violate state law, including Florida Statute 760.27, and may result in denial of service or revocation of a previously issued letter.
6. Payment & Refunds
Fees are due at the time of booking. If a physician determines a letter cannot be issued following your evaluation, you will receive a full refund of the visit fee. Refunds are not available once a letter has been issued.
7. Letter Validity & Acceptance
We format every letter to meet current Fair Housing Act and ADA documentation standards. However, we cannot guarantee acceptance by any specific landlord, airline, or institution, as third parties retain some discretion under the law.
8. Intellectual Property
All content on this website, including text, graphics, and logos, is the property of ESA Letter For Housing and may not be reproduced without permission.
9. Limitation of Liability
To the maximum extent permitted by law, ESA Letter For Housing is not liable for indirect, incidental, or consequential damages arising from use of our services, including denial of a letter by a third party.
10. Governing Law
These terms are governed by the laws of the State of Florida, without regard to conflict-of-law principles.
11. Changes to These Terms
We may revise these terms periodically. Continued use of our services after changes are posted constitutes acceptance of the revised terms.
