1. Our Commitment
ESA Letter For Housing is committed to protecting the privacy and security of your Protected Health Information (PHI) in accordance with HIPAA and its implementing regulations.
2. What Counts as PHI
PHI includes any information about your health condition, treatment, or history that could reasonably identify you, including notes from your telehealth visit and your signed letter.
3. Administrative Safeguards
Access to PHI is limited to staff and physicians who need it to provide your care. All team members complete HIPAA training, and access is logged and regularly reviewed.
4. Technical Safeguards
We use encryption in transit and at rest, multi-factor authentication for staff accounts, and secure, HIPAA-compliant video visit software for telehealth appointments.
5. Physical Safeguards
Our systems are hosted with vendors that maintain HIPAA-compliant data centers with restricted physical access, redundant backups, and continuous monitoring.
6. Business Associate Agreements
Any third-party vendor that may handle PHI on our behalf — such as our scheduling and video visit platform — operates under a signed Business Associate Agreement (BAA) as required by HIPAA.
7. Your Rights Under HIPAA
You have the right to access your records, request corrections, request an accounting of certain disclosures, request restrictions on how your information is used, request confidential communication, and file a complaint if you believe your rights have been violated.
8. Breach Notification
In the unlikely event of a breach involving your PHI, we will notify affected individuals in accordance with HIPAA's breach notification requirements.
